Death Claims

 

Overview

Information on how to file a claim (if you are a beneficiary) or a Family Option-C claim (if you are an employee or annuitant who elected this optional coverage) can be found here. These pages will walk you through the process of reporting the death of someone covered by the Federal Employees' Group Life Insurance Program.

Checking Status of a Life Insurance Claim

If it has been at least 30 days from the date you submitted your claim form, you may call 1-800-633-4542 (between the hours of 8:30 am - 4:00 pm Eastern Standard Time, Monday - Friday)

and talk to a customer service representative of the Office of Federal Employees' Group Life Insurance. That is the office that pays the life insurance claims. Overseas beneficiaries should call 212-578-2975.

Be sure you have the following information ready when you make the call:

  1. the name of the insured employee/retiree/compensationer
  2. the insured's social security number
  3. the name of the deceased (if different), and
  4. the date of death of the deceased.

Reporting Deaths

These pages will walk you through the process of reporting the death of someone covered by the Federal Employees' Group Life Insurance Program. Start by choosing the category below which covers the person who died.

Employee (Or Employee's Family Member)

You must report the death to the human resources office of the employee's employing agency. Be sure to have the employee's full name and social security number. You'll also need the deceased's date of death.

The agency will send you (and anyone else who appears eligible for life insurance benefits) the life insurance claim form.

Be sure to read the instructions on the form. You will have to print out the claim form and give or mail it to the employing agency, with a certified copy of the death certificate.

Annuitant

Death of Annuitant

You can report the death in one of three ways:

  1. ONLINE:

  2. PHONE:
    Call 1-888-767-6738 (1-88USOPMRET).

    The phone lines are open from 7:50AM to 5:00PM. It is a busy phone number so you may want to try early in the morning or after 4:00PM when the phone lines are less busy.

    Include the deceased annuitant's full name, name of deceased, date of death, retirement claim number and/or social security number.

OPM will send you (and anyone who appears eligible for life insurance benefits) the life insurance claim form (FE-6).

IF YOU WISH, you may complete the claim form FE-6 ahead of time by downloading the form from this website.

Be sure to read the instructions on the form. You will have to print out the FE-6 and send it, with a certified copy of the death certificate, to the Office of Federal Employees' Group Life Insurance, P.O. Box 6080, Scranton, PA 18505-6080.

This report of death will also stop the monthly annuity payments. OPM will send the appropriate forms for claiming a survivor annuity or a lump-sum payment of retirement contributions, if applicable, and take any necessary action on health benefits.

Death of Family Member

Complete the claim form FE-6DEP by downloading the form from this website.

Be sure to read the instructions on the form. If you need help, you may contact the Office of Federal Employees' Group Life Insurance (1-800-633-4542). You will have to print out the FE-6DEP and send it, with a certified copy of the death certificate, to:

When we receive the FE-6DEP for the death of a spouse, we will also eliminate the annuity reduction for a survivor benefit, if applicable. You do not need to request this separately.

The Annuitant Receives Monthly Payments From Another Retirement System

You must report the death to that retirement system. Be sure to have the annuitant's full name, social security number and retirement claim number, if any. You'll also need the deceased annuitant's date of death.

The retirement system will notify OPM of the death. OPM will send you (and anyone who appears eligible for life insurance benefits) the life insurance claim form.

For Family Member's Death

Download the Form FE-6DEP. Be sure to read the instructions on the FE-6 DEP form. If you need help, you may contact the Office of Federal Employees' Group Life Insurance (1-800-633-4542). You will have to print out the FE-6 DEP and send it, with a certified copy of the death certificate, to:

Compensationer

Death of Person Receiving Compensation Payments

The person receiving compensation payments has received the payments for less than 12 months AND is still on an agency's rolls as an employee

You must report the death to the human resources office of the employee's (compensationer's) employing agency. Be sure to have the employee's (compensationer's) full name, social security number and compensation claim number. You'll also need the deceased's date of death.

The agency will send you (and anyone else who appears eligible for life insurance benefits) the life insurance claim form.

Be sure to read the instructions on the FE-6 or FE-6DEP. You will have to print out the form and give or mail it to the employing agency, with a certified copy of the death certificate.

Death of Person Receiving Compensation Payments

The person receiving compensation payments is still NOT on an agency's rolls as an employee OR has been receiving compensation payments for 12 months or more

You can report the death in one of three ways:

    Include the deceased compensationer's full name, date of death, and social security number.

Claiming FEGLI Life Insurance Benefits

To claim FEGLI benefits for the death of an employee or annuitant:

To claim FEGLI benefits for the death of a family member of an employee or annuitant:

FE-6 DEP -- Statement of Claim, Option C

These forms are screen-fillable Adobe Acrobat PDF files. Using the free downloadable Adobe Acrobat reader you may complete the form on the screen and sign it. Note, however, that if you are using the free Acrobat reader software you cannot save the form with the information that you have typed in it on the screen. You need the commercial Adobe Acrobat program in order to save the form with the data.

Be sure to read the instructions on the form. You will have to print out the claim form. It cannot be submitted on-line.

If you or someone else hasn't done so already, you also need to report the death.
