  1. How do I report my bankruptcy?

    If you declared a bankruptcy during the preceding calendar year, you should report it in the Bankruptcy section (Section 12). Report the amount, bankruptcy type, date and place filed. Also, provide an explanation within the Explanation field as to why the bankruptcy was filed.

    Enter this information in the "Other Liabilities" section.
