Model Pledge Card

The 2002 model CFC pledge card has been posted on the CFC website for downloading and reproduction. Please note that there are two important changes to the document this year. First, the Office of Personnel Management (OPM) has assigned a form number, OPM Form 1654, which must appear on the face of the document along with the date of the revision, April 2002. Second, there have been revisions to the Privacy Act Statement, which must appear on the back of all pledge forms.

Campaigns must use this model pledge form as the basis for designing pledge forms locally. While campaigns may incorporate additional giving levels and include their award recognition programs, no further modifications are permitted unless approved in advance by OPM. Such requests should be faxed to OPM at 202/606-5056.

Local Appeals

In accordance with the CFC regulations, organizations that are denied participation by the Local Federal Coordinating Committee (LFCC) on appeal have the right to appeal to the Dan G. Blair, Deputy Director of the Office of Personnel Management. Due to mail delays caused by irradiation of materials sent to Federal buildings via the United States Postal Services, we request that you include the following language in all appeal denial letters:

Please note that OPM is still experiencing significant delays with mail service associated with irradiating mail prior to its delivery to Federal offices. As a result, all organizations submitting an appeal are vigorously encouraged to use "next day" or "overnight delivery".

Your cooperation in this matter will greatly assist in OPM's effort to receive and review local appeals in a timely manner.
